New Patient Registration

To register you need to come in to the surgery and complete a GPR (registration) form. However, you can start the process by filling in the following New patient questionnaire and sending it OR by bringing it when you come to register.

It is part of the terms & conditions of living in student halls that you register with a practice covering that area.

Please note: If you use the online form you are not fully registered until you come into the practice to sign the relevant documents.

Please inform us at once if you change your address or mobile number.

Also, please note that updating your details with University administration will not update your records at our practice.
